By registering this form you agree to the following terms: I/WE, being the parent(s) or guardian(s) of the named player do hereby consent to the said child participating in all the activities of this hockey program and do hereby release, absolve and indemnify ES Hockey Training (Owners and Staff) from any claim which I/We, or the said child may have as a result of participation. I/We do assume all risks and hazards incidental to the above article and hereby waive all claims whatsoever against ES Hockey Training. I/WE understand that all participants must wear full CHA approved hockey equipment during all on ice activities. ES Hockey reserves the right to to use any photographs and/or video taken of applicant for advertising and/or promotional purposes.
